Late secondary osteoplasty is still a very important procedure despite improvements in the primary treatment of LPS. The procedure can be performed either alone or in combination with maxillomandibular osteotomy. It consists in the grafting of autologous bone (iliac crest) at the level of the alveolopalatine cleft so as to assure stability of the maxillary stumps and to achieve appropriate alveolar height. The authors suggest the possibility of performing late secondary oesteoblasty using a gelatin and glycerol-based granular ceramic hydroxylapatite (Osprogel R), listing the characteristics of the material and describing the preliminary results obtained in 5 patients treated for the after-effects of monolateral LPS by Osprogel (R) bone graft and followed up for a period of at least one year. The results of the study show that the procedure is indicated in carefully selected patients with small-sized monolateral clefts and stable occlusion.
